2008 “Entry Form”
Read this package carefully.
When you are ready to fill out the form, select the

Select the calypso/songs on your record, CD or tape you would like considered for nomination.
Select four (4) categories from the categories list.
Proceed to complete the “entry form.” Begin by filling in your name, address and phone number in the blank space provided.
Select the category from the drop down list.
Complete the rest of the form.
When you have completed the “entry form,” hit
your browser's PRINT icon or just print the form. Sign and date it. Mail your “entry form” along with your record, CD or tape, a copy of your biography and a picture of yourself to one of the addresses provided under the Requirements for Eligibility before ----
